If your old passport contains a residence permit (also known as leave to remain), you must transfer that residence permit to your new passport when the old one expires. You can choose not to have the residence permit transferred to your new passport, but in that case, you must carry both your old passport and your new passport when you travel to the United Kingdom, as evidence of your resident status and if you cannot produce your original permit, you may not be allowed to enter the country.
The Transfer of Condition cannot be done at passport control and must be carried out through an application to the UK Border Agency - the applicant must currently be in UK residence and must have valid permission to stay in the UK.
